Payment for all exams, evaluations, consultations and periodontal maintenance appointments are always due in full at the time of services rendered as well as services not included within a treatment plan such as x-rays and diagnostic procedures.
You are welcome to pay for your office visits and treatments by cash, check, credit or debit card at the time of service.
We are not listed within the network of participating providers for any insurance plans. Therefore as a courtesy, we will be happy to submit claims to your dental insurance on your behalf for each visit to utilize your out of network benefits. With out of network benefits, your insurance company will issue the payment directly to you as a reimbursement since you have paid for services in full at our office.
We realize the financial impact of unexpected or even extensive planned dental procedures can bring to a household budget. For this reason, we are more than happy to set up a payment plan for the more extensive periodontal procedures that are included within your personalized treatment plan. Patients with dental insurance benefits can enjoy the ease of paying as little as 30% of the treatment plan amount as a down payment as we await determination of benefits from your insurance company. A payment plan will be offered for any remaining balance on your account after your benefits have been utilized. This "waiting period" for determination could last up to 30 days giving you the time you need to financially recoup as we finance your account with no interest for up to 60- 90 days.
